Primary Care Partners offers a true "medical home" for Western Colorado and Eastern Utah patients. This means physicians can help them navigate the complex world of medical care; providing them with support and reassurance. Four family physician groups, a pediatrics group, diagnostics, nutritional services and diabetic education; an after-hours care clinic, physical therapy and sports medicine round out our services. Also embedded within the family practice groups, are physician-led care coordinators who help chronically-ill patients manage their care; as well as behaviorists who can address emotional/mental problems that block progress in managing a patient's health. Our Patient Portal offers online services such as scheduling appointments, the ability to ask medical questions of their provider team, and more.
