Nighttime hip pain is one of the most common complaints we see at R3 Physio—and one of the most frustrating for people to live with. When you can’t get comfortable in bed, everything suffers: your sleep quality, your recovery, your mood, and even your energy the next day.

Here’s what most people don’t realize: nighttime hip pain rarely starts in the hip itself. Many times, the deeper issue is in the pelvis, the low back, or the fascia that connects them.

Your hips and pelvis work as a team. When one area becomes tight, restricted, or misaligned, it forces the rest of the system to compensate—especially when you lie down and your body finally relaxes into gravity.

A pillow between your knees can help because it restores neutral alignment. It decreases the rotation pull on the pelvis and calms tension in the muscles and fascia surrounding the hip joint. But while this can help you sleep better tonight, it doesn’t address the root cause.

At R3 Physio, we look at:

  • How your hip joint is positioned and whether the capsule is restricted

  • Whether tight pelvic fascia is pulling your hip out of alignment

  • How your low back and sacrum are moving

  • Whether organ restrictions (like the intestines or bladder) are affecting pelvic mechanics

  • How your nervous system is responding to nighttime positioning

This whole‑body evaluation helps us uncover why you’re hurting—and more importantly, how to help you sleep pain‑free again.

If nighttime hip pain is disrupting your rest, your body may be asking for deeper support.
