Create an article.Case management for seniors can make a complicated care journey easier to understand. When an older adult moves between a hospital, rehabilitation center, physician’s office and home, families in Smyrna, Vinings and surrounding South Cobb communities may suddenly face unfamiliar decisions, appointments and instructions.

National Case Management Week, October 11–17, recognizes professionals who help people coordinate care and connect with appropriate resources. Understanding their role can help families ask better questions and prepare for safer transitions.

What is case management for seniors?

A case manager helps organize information, services and next steps around a person’s needs. Depending on the setting, the role may be filled by a nurse, social worker or another qualified professional. Case managers do not replace physicians or make every decision for a family. They help the care team and family communicate more clearly.

How case managers can help families

Case managers may review discharge instructions, help schedule follow-up care, explain available community resources, identify equipment or transportation needs, and clarify who is responsible for each next step. They may also help families understand insurance or benefit questions within the scope of their role.

Hospital-to-home transitions deserve a plan

The first days after discharge can be especially demanding. An older adult may have a new medication schedule, activity restrictions, therapy appointments or instructions that differ from the routine before hospitalization.

Before leaving, ask:

  • What has changed in the medication list?

  • Which symptoms require an urgent call?

  • Who schedules follow-up appointments?

  • Is transportation arranged?

  • What equipment or supplies are needed at home?

  • Who can help with meals, personal care and household routines?

Where non-medical home care fits

A case manager coordinates clinical and community resources. A non-medical caregiver supports everyday life at home. That support may include companionship, meal preparation, transportation, errands, light housekeeping, personal care and medication reminders.

The roles are different, but they can complement each other. A clear care plan helps everyone understand boundaries and responsibilities.

Prepare for the conversation

Bring an updated medication list, contact information for all providers, a calendar, insurance details and written questions. If possible, choose one family member to collect information and share updates with the rest of the family.

Frequently asked questions about case management for seniors

Does every older adult need a case manager?

No. Case management may be especially useful when care is complex, several providers are involved, or a transition between settings is difficult to manage.

Is a case manager the same as a home caregiver?

No. A case manager coordinates services and information. A non-medical caregiver provides hands-on support with approved daily activities in the home and community.

What should families ask before discharge?

Ask for written instructions, medication changes, warning signs, follow-up appointments, equipment needs and the name and number of the person to contact with questions.
