Conejo Valley Primary Health Care Delivers Customized Care for Midlife Health Concerns

When Lynn Coates‑Leisen, MSN, FNP‑C, MSCP, first began working in a practice that offered hormone therapy, she noticed something striking: Women and men experienced profound improvements when their hormone needs were addressed. The experience sparked a deep curiosity that led her down a 12‑year path of advanced hormone education, clinical practice and a commitment to filling what she describes as “a huge gap in women’s care.”

Today, Lynn serves as Nurse Practitioner and CEO of Conejo Valley Primary Health Care (CVPH), a practice built on the belief that healthcare should feel personal.

“Patients deserve to be heard, respected and supported—not rushed or dismissed,” she says. That philosophy guides every aspect of the clinic’s approach to primary care for women and men across all stages of life.

CVPH offers individualized care with a distinct focus on women’s midlife health, including perimenopause, menopause, thyroid concerns, weight management, PMOS, low libido and proactive cardiovascular care. For women navigating perimenopause or menopause, the practice takes time to understand the full picture—whether symptoms include hot flashes, sleep changes, mood shifts, pelvic discomfort, weight concerns or simply not feeling like oneself. Many women arrive after years of being told their symptoms are “just part of getting older.” At CVPH, those concerns are taken seriously.

“We listen without judgment,” Lynn says. “We validate what women are experiencing and work to understand what may be contributing to the changes they’re feeling.” Treatment plans may include lifestyle support, nonhormonal options, bioidentical hormone therapy or testosterone when appropriate. Each plan is tailored to the individual’s health history, goals and long‑term wellness.

A Unique Expertise in Midlife Care

CVPH stands out in Ventura County for its specialized expertise. Lynn is the first healthcare provider in the county to earn the Menopause Society’s MSCP certification. She also holds advanced hormone‑therapy training from Worldlink Medical, an education group for physicians and advanced practitioners.

As a family nurse practitioner, Lynn is trained to see the “big picture”—how hormones intersect with past, present and future health. She is committed to ongoing education in women’s midlife care, a rapidly evolving field.

“It’s not unusual for women to see three to six providers before they find someone who can help them,” she notes. Her mission is to break the cycle of dismissal and denial that so many women experience.

Comprehensive Services for Women and Men

CVPH offers a wide range of services, including

  • Perimenopause and menopause consultations
  • Bioidentical hormone therapy
  • Testosterone therapy for appropriate patients
  • Personalized primary care for women and men
  • Preventive health visits and chronic‑condition management
  • Cardiovascular risk assessment
  • PCOS and thyroid evaluation
  • Medical weight management
  • Low‑libido evaluation
  • Men’s health services
  • Lifestyle guidance tailored to individual goals

A Philosophy Rooted in Trust and Personalization

The practice’s core philosophy is simple: Primary care should be personal, proactive and grounded in trust. Every patient brings a unique story, and CVPH aims to identify root causes rather than simply adding medications to treat symptoms. Long‑term relationships are central to this work. Lynn has cared for many patients for more than a decade, giving her a deep understanding of their health history, family circumstances and what has—or hasn’t—worked in the past.

That continuity creates a two‑way partnership. Patients become more invested in their own health, more comfortable raising concerns early and more confident in making informed decisions. For women in midlife, this history is especially valuable, allowing earlier recognition of changes in sleep, mood, energy, menstrual patterns, weight, libido or cardiovascular and thyroid health.

Expanding Access and Advancing Care

CVPH is also taking steps toward becoming one of California’s first independent nurse practitioner clinics. Lynn is among the first 104 NPs in the state qualified to practice independently, reflecting years of clinical experience and a commitment to expanding access to high‑quality primary care.

Looking ahead, the practice may add services such as body‑composition scanning and pelvic‑floor and incontinence care to further support women through midlife and beyond.

Changing the Conversation Around Menopause

Menopause and hormone education remain central to CVPH’s mission. Lynn emphasizes that menopause deserves the same level of clinical understanding as any other major health condition.

“Women make up just over half the population, and nearly half are older than 45,” she says. “Millions deserve informed, respectful care through this stage of life. Dismissing their symptoms is not acceptable.”

At Conejo Valley Primary Health Care, that change is happening—one woman at a time.
