Lasers in the Desert

New technology and skilled technicians can work magic when done correctly

A detailed conversation with Danielle Moser, PA-C, owner of Facial Artistry & Aesthetics.

1. How does CoolPeel laser work and how does it differ from traditional resurfacing?
CoolPeel is a fractional CO2 laser that creates microscopic skin channels, stimulating collagen and elastin with minimal thermal damage. Unlike traditional ablative CO2 lasers, which cause more heat-related trauma, CoolPeel achieves meaningful rejuvenation with significantly less downtime. Results are gradual, typically requiring 3–4 sessions.

2. When combining CoolPeel and IPL, what is the recommended sequence and timing?
IPL is performed first to target pigment and vascular concerns, followed by CoolPeel to improve texture and stimulate collagen. Sessions are typically spaced 2–4 weeks apart; more aggressive treatments may need 4–6 weeks. In select cases, both may be done in one session.

3. Which pigmentation types respond best to IPL, and what are the contraindications?
IPL works best on epidermal pigment—solar lentigines, freckles, and diffuse photodamage. Melasma is generally a poor candidate due to risk of recurrence. Fitzpatrick skin types I–III are the safest candidates; darker skin types carry increased risk of hyperpigmentation and burns, making pre-treatment assessment essential.

4. How should treatment plans be adjusted for darker skin tones?
Use conservative settings, fewer passes, and longer intervals between sessions. IPL should be reduced or avoided if melasma risk is high. CoolPeel is used in lower-energy, superficial modes with strict endpoint control. RF microneedling is preferred since it bypasses epidermal melanin entirely. Pre- and post-treatment pigment suppression—hydroquinone alternatives, tranexamic acid, sun avoidance—is standard.

5. How does RF microneedling compare to CoolPeel for collagen stimulation?
RF microneedling delivers radiofrequency heat deep into the dermis via insulated needles, making it superior for skin laxity, acne scars, and structural tightening. CoolPeel works more superficially, targeting sun damage, pigmentation, and tone. Many patients benefit from both—RF microneedling first, then CoolPeel.

6. Why combine IPL with RF microneedling rather than using either alone?
IPL addresses superficial concerns like pigmentation and redness, while RF microneedling improves deeper concerns like laxity and scars. Together they provide more complete rejuvenation, with outcomes superior to either treatment alone.

7. How many sessions are needed, and how long do results last?
Most patients see improvement after 1–2 sessions, with optimal results after 3 treatments spaced 4–6 weeks apart. Collagen remodeling continues for 3–6 months post-treatment. Results typically last 1–2 years with proper sun protection; annual maintenance helps preserve outcomes.

8. What pre-treatment protocols should patients follow?
Avoid sun exposure, tanning, and self-tanners for 2–4 weeks prior. Use daily broad-spectrum SPF. Discontinue retinoids and exfoliating acids 5 days before treatment. Inform your provider of photosensitizing medications, active infections, or recent procedures.

9. For jawline and neck laxity, how do RF microneedling and CoolPeel compare—and can they be combined?
RF microneedling is more effective for jawline and neck laxity, targeting the dermis and subdermal tissue where tightening occurs. CoolPeel primarily improves surface texture with modest tightening. Since they treat different layers, the two can be safely combined.

10. What post-treatment skincare best supports recovery?
Focus on healing, hydration, and collagen support. Hyaluronic acid restores moisture and barrier function; peptides and growth factors promote collagen synthesis. Vitamin C, once healed, reduces oxidative stress. Niacinamide, tranexamic acid, and tyrosinase inhibitors help control pigment issues. Daily SPF is essential.

11. What can patients do to heal faster and enhance results?
We recommend Ariessence Pure PDGF+ applied immediately post-treatment to accelerate healing, reduce redness, and boost collagen. About two weeks later, SkinVive skin hydrator provides deep hydration—improving smoothness and softening fine lines for up to 6 months.
